Warm welcome to Teresa Ward, Community Care Coordinator

Submitted by the San Juan County Hospital District #1

Teresa Ward, RN, joined Village at Home’s team in August 2025 as the Community Care Coordinator. During September, she worked closely with Stephanie Zamora, Community Paramedicine EMT, to care for patients and to work on the transition of Community Paramedicine from San Juan Island EMS to Village at Home.

Teresa has a long history in healthcare. She has been a nurse for over three decades. Her experience includes intensive care, adult and pediatric medicine, clinical research, and, most recently, academia. Teresa is looking forward to caring for patients and their families and partnering with community members to ensure that island residents are receiving the best care possible. Teresa is grateful to Stephanie Zamora, EMT, and the entire San Juan Island EMS team for their time, care, and knowledge, which have been instrumental in helping Teresa prepare for her new role.

Community Paramedicine bridges the gap between healthcare providers and the resources people need to stay in their homes, helping to avoid using 911 and the Emergency Room (ER) for preventable healthcare services and enabling people to get on the road to better health while keeping our first responders’ call volume lower.

Both Village at Home and San Juan Island EMS are part of San Juan County Public Hospital District No. 1. By transitioning from San Juan Island EMS to Village at Home, Community Paramedicine will be able to grow with our community and work hand-in-hand with Village at Home to ensure a consistent and compassionate continuum of care.
