To better understand the current need for vaccinations from the San Juan County 65+ community, San Juan County Health and Community Services asks that those who meet ALL of the four measures below fill out a simple form.

Please be:

• A San Juan County resident (short term or long term), and

• Age 65 or older, and

• Currently UNVACCINATED, and

• Interested in being vaccinated.
